From your first phone call (or email!) to us, our goal is to make you feel welcome, fully informed of what is going on, and actively involved in your treatment and care. We know contacting us took courage, but now you’ve taken your first step toward a more gratifying life. Well done, you!

Whatever brought you here, we’re ready to share the support and tools you’ll need to navigate life’s milestones so you can find purpose and happiness.

But first, you have a little bit of homework.

Please complete the appropriate New Patient Form (forms found below) at least 24 hours before your appointment. This gives our staff time to review your background before we meet you. Also, you’ll be added to a cancellation list that will possibly move your appointment up sooner:

Please note, the above list is not all-inclusive, as each individual clinician is also contracted with other insurance carriers.

We file insurance as a courtesy to our patients. Any charges accrued in this office are your responsibility regardless of insurance pre authorization or estimated amounts. You (not your insurance company) are responsible for full payment of all fees.

It’s possible your insurance company may not pay for your visits with Milestones if you see another mental health provider on the same day or week. If this occurs, you will be responsible for any charges. Therefore, please do not schedule more than one mental health appointment per week.
